Healthcare Middleware
We linked a human service system to a a local Health Information Exchange, so patients gets quicker care without repeating their personal information.
The original press release describing this effort is here. We developed the open source middleware which enables this sharing. Below is a screen capture of the referral in the healthcare system, which received the data via HL7 Continuity of Care Document secure web messages. Recently, we added the referral codes and description to the information passed.
